How do I register with the Practice?

If you do not wish to attend the surgery, but wish to register with the Practice, then please click here. You will then see a simple form to complete which is automatically sent to us by e-mail.

If you wish to register by attending the surgery, please bring your medical card; the receptionist can help you complete it. If you do not possess a medical card please ask at reception for an application form. If you would like to meet the doctor before registering, please make an appointment.

Temporary residents are registered on a special form provided at reception.

After registration you may see any doctor (regardless of with whom you are registered) but, once your choice is made, it is preferable to remain with that doctor.

New patients will be invited to complete a simple personal health questionnaire and make an appointment with the practice nurse for a basic health screen and discussion of any health worries. If you take repeat medication, your appointment should be with your registered GP rather than the nurse.

Private Patients

Patients who do not wish to be registered as an NHS patient may request to be a private patient by liaison with an individual doctor. It is, however, the Practice policy that all patients are treated equally and that an NHS patient will receive at least as good a service as private patients.
